New Patient Information
Forms
For your first treatment please download the new patient information and forms.  It contains our Welcome Letter, Notice of Privacy Practices (HIPPA), Acknowledgement of Receipt form (HIPPA), Informed Consent to Release Information form (HIPPA), our Intake Form, and the Consent to Treat form. Please print and look over these pages. If you like, fill them out before you come to the clinic, or fill them out at the clinic 15 min. 
before your treatment.  All downloads are in PDF format. 
We Take Cards
We perfer cash due to the transaction fees that cut into are already skinny margins. 
You Decide How Long Your Acupuncture Treatment Lasts  
Plan on being in the clinic 90 minutes for your first treatment. Following treatments may not be as long. In community acupuncture we don't push you out to make room for the next patient. We have enough lounge chairs to give you the length of treatment you feel comfortable with. Many people relax into a restful sleep during their treatment, so if you need to wake at a certain time, let us know and we will come and remove your needles. 
Treatment Plan
After your first treatment the acupuncturist will let you know how many treatments are recommended for your condition, and how frequently you should you should schedule your appointments. Following an individualized treatment plan, you determine what you can pay on the sliding scale. All following treatments you make your next appointment and pay before treatment, which allows you to simply leave the clinic when you have had your needles removed.

On the day of your appointment
Your time is valuable. We ask that you please be on time for your first appointment. If you are more that 20 min. late then we may need to reshedule it. 
  Please no pets. Only service dogs are allowed in the clinic. 
  Eat a bit of something before the appointment. Acupuncture is not recommended on an empty stomach.
  Please do not wear scented lotions, perfumes, or cologne. Some patients are sensitive to smells and can trigger migraines, others are allergic to scents.
  Wear loose comfortable clothing that will not bind you. You should be able to roll up your sleeves above your elbows, and pants above your knees.  
  Following your treatment do not participate in strenuous activities. Moderate activities such as walking, gentle yoga, or tai chi are beneficial.
  The treatment area has a quiet atmosphere. If you desire, bring your own music and listen with headphones or earbuds. Music should be gentle and relaxing.
